Family Consent Form

I hereby give permission for my family member(s) to attend the Chinese Language and Cultural School organized by the Chinese Cultural Association of New Brunswick (herein referred to as CCANB) and participate in the various activities, and events in connection therewith. I am aware in signing this document for my family member(s) and/or myself’s participation in the CCANB Chinese Language and Cultural School and all its events and activities that certain risks and/or dangers may exist, including loss and/or damage to personal property, injury and/or fatality due to slipping, falling, falling objects, or suffering from any type of accident, illness, or other causes. I acknowledge that while CCANB through its agents, volunteer organizers and/or employees, will make every reasonable effort to instruct my family member(s) properly to minimize exposure to known risks all danger (hazards and perils) associated with these activities cannot be foreseen. I have instructed my family member(s) that it is his/her personal duty and responsibility to learn and to follow the safety standards, guidelines, rules and procedures established by CCANB’s organization team and will make them aware of any point during the activity in which my family member(s) question his/her knowledge or these standards, guidelines, rules and procedures or his/her ability to participate.

In checking this box, I understand and assume all risks and dangers (hazards and perils) associated with all the activities and events organized by CCANB in which my family member(s) and I will be participating in and waive all claims or causes of action arising therefrom and hereby release all persons or agents from liability whether cause by negligence, breach of contract, strict liability or otherwise, which I may or ever have against CCANB, its successors and assigns, its Board of Directors or Trustees, its officers, agents, volunteers, employees, and their heirs, executors and assigns.

Furthermore, I give my consent to CCANB and members of its organization team and/or other medical personnel to treat my child in an emergency situation.

Chinese School Code of Conduct

In order to create a safe learning environment, the following school rules must be observed by all students and parents/guardians and will be strictly enforced by teachers and school administrator. All prospective students and their parents/guardians will be required to understand and sign the following consent agreement at registration.

  • Student should arrive on time. Follow the class rules.
    按时上课,遵守课堂纪律。
  • Students should follow the teachers’ and school administrators’ instructions.
    听从老师、学校管理人员的指导。
    Students should be at the classroom on time and not leave early.
    不迟到,不早退。
    Students should let the teacher know in advance when he or she cannot attend the class.
    因故请假,应提前通知老师。
  • Students should finish homework on time and study Chinese at home.
    课后认真学习中文,按时完成作业。
  • Students should finish protect the school properties. No personal use of school property without permission. No doodling on desks or walls. Parents will be asked to pay for any property damages caused by the student.
    爱护公共设施和公共财产。未得许可不得动用学校的物品。不能在桌子和墙上涂鸦。如有损坏公物必须按市价赔偿。
  • Under normal circumstances, snacks are not allowed in the classroom. If necessary, students can take breaks and eat snacks in the designated areas by the teacher during classes. They should also keep the designated areas clean when they finish.
    一般情况下,在学校不得吃零食。如有需要,只能在老师指定的范围内休息和吃零食,吃完后打扫好清洁。
  • Running or yelling in any classroom, hallway or stairway is not permitted.
    不在教室、走廊和楼梯奔跑,不大声喧哗。
  • Student should keep classroom clean and without litter.
    保持教室干净整洁,不乱扔纸片。
  • Students should be friendly to their classmates.
  • The following are ZERO TOLERANCE behaviors and will result in the immediate suspension from the school with no refund:
    • Use of alcohol or drugs.
      饮酒或吸毒
    • Verbal or physical abuse of other participants or staff.
      言语或身体虐待其他同学或工作人员
    • Stealing or damaging St. Thomas University property or personal property of other participants or stuff.
      盗窃或损坏 St. Thomas 大学的公共财产或学校其他同学或工作人员的个人财产。

Media Consent Form

I hereby authorize Chinese Cultural Association of New Brunswick, hereafter referred to as “CCANB,” to publish photography, video or audio recordings of myself and/or the minor child or children listed below, and our names and likenesses, for use in the CCANB’s print, online, and video-based materials, as well as other publications.

I hereby release and hold harmless on CCANB from any reasonable expectation of privacy or confidentiality for myself and for the minor child and children listed below associated with the photos taken by CCANB. Further, I attest that I am the parent or legal guardian of the child or children listed below and that I have full authority to consent and authorize CCANB to use their likenesses and names.

I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children, will receive financial compensation of any type associated with the taking or publication of these photographs or participation in marketing materials or other publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.

I hereby release CCANB, its volunteers, employees and any third parties involved in the creation or publication of publications, from liability for any claims by me or any third party in connection with my participation or the participation of my family members.
